NYS resident, who qualifies for Medicaid and is elderly, has a disability, or requires assistance with activities of daily living, or needs skilled care. Must be medically stable and capable of overseeing their own care or have a designated representative.

Description

* Provides access to support programs available through the New York State Department of Health including: Children's Health Home: care management addressing chronic medical, mental health, substance abuse, behavioral health, and developmental needs. Consumer Directed Personal Assistance (CDPAP): a Medicaid service that allows people with chronic conditions with or without developmental disabilities who need aid or nursing care to hire and train attendants to provide care.
