Eligibility
Youth ages 5-17 who live in Eastern Suffolk County, have Axis 1 primary mental health diagnosis, history of psychiatric hospitalization, a stable home environment and are at risk of psychiatric hospitalization.

Prevents long term psychiatric hospitalization and out of home placement for youth who are living in the community and have significant mental health needs that cannot be met by traditional community programs. Provides core services including Intensive Care Coordination-assessment, goal planning, linkage to community resources and coordination of service plan to help youth and family function in their natural environments; and Crisis Response for 24-hour call intervention. Referrals are made through SPOA (Single Point of Access). Services are provided in the home and community of the client.
