Ages 5-21 who are diagnosed with a serious emotional disturbance (SED) diagnosis, complex trauma, or have two chronic conditions. Child must have active Medicaid. Program serves Nassau County and portions of Queens and Suffolk. Call the program for referral information.

Description

Provides comprehensive, integrated, behavioral health and medical case management to children ages 5-21 and their families to ensure access to appropriate services, improve health outcomes and prevent hospitalizations. Services are provided in the home, school and/or other community setting with frequency and duration depending upon the need of each child and his or her family.
