Provides primary care medical, dental, behavioral health services for all ages. As a Federally Qualified Health Center, the practice serves uninsured, underinsured, and underserved individuals. Services include: Well-baby check ups. Immunizations, women's health, asthma/allergy care, vision screening, preventative medicine. Behavioral Health for medical patients. Physical exams for work and school. Blood pressure screening. Preventive medicine. Dental exams, x-rays, prophylaxis, fillings, extractions, root canal therapy, crown and bridge, dentures, sealants. Certified application counselor available to assist individuals with their New York State of Health Marketplace Insurance enrollment.
