Designed to help individuals recovering from mental illness and/or addiction improve their health and goals. Services provided in the home and community based settings which include habilitation, psychosocial rehabilitation, peer support services, pre-vocational employment, education support services, and family support and training.

Provides housing and services for Long Island residents with very low incomes who are recovering from mental illness or living with HIV/AIDS or other disabilities. Also provides care coordination services to help adults and children living with chronic health conditions access health care, housing and community resources.
