Description

Provides supported housing, through varies programs, for individuals diagnosed with severe mental illness, chronic chemical dependency or other disabling conditions. These programs give the opportunity for these individuals to transition from homelessness to affordable, stable, and permanent supported housing. Participants are required to actively work on their service plans to develop self sufficiency.
