Description

In collaboration with BestSelf Behavioral Health, serves high-risk incarcerated individuals with co-occurring mental illness and substance addiction. Participants work weekly with a case manager and peer support specialist to assist in the development of case plans and can engage in any service offered directly through BestSelf or via linkage to other community-based organizations, including housing, education and employment services, mental health and addiction services, public assistance, and more. Case management continues for up to one year following release. By building relationships and connecting individuals with supportive services pre-release, the New Dawn Initiative seeks to increase the likelihood individuals will remain engaged in services and successfully re-enter and remain in their communities. ##REENTRY##
