Eligibility
For NHTD Waiver: must have active Medicaid, aged 65+ or 18-64 with a chronic physical disability, must meet Nursing Home level of care with an assessment score of 5 or more.
For TBI Waiver: must have active Medicaid, ages 18-64 (if 65+ apply to NHTD), must need Nursing Home Level of Care with an assessment score of 5 or more, have a documented Traumatic Brain Injury.
