Eligibility

Individuals ages 18 years and older who:
  • have a primary diagnosis of serious mental illness
  • are in the process of relocating from a state-funded program (supervised site, hospital, correctional facility)
  • are experiencing substantial impairments in function
  • are homeless
  • have high Medicaid costs or are under an AOT order
  • have the capacity to live independently

Monday - Friday, 8:30 AM - 4:30 PM

Complete a Single Point of Access form (SPOA) through Onondaga County

Provides housing support to eligible individuals with a serious mental illness diagnosis. Services may include housing advocacy, and housing search assistance, followed by furnishing, household supplies, and security deposit when necessary.
