Must meet the follow 4 criteria:
1 - Resident of Orleans County
2 - Enrolled in Medicaid* - individuals dually enrolled in Medicaid/Medicare are also eligible
3 - Two or more chronic health conditions (such as asthma, diabetes, mental health condition, or substance use disorder), or a significant mental illness, or living with HIV/AIDS
4 - Significant behavioral, medical, and/or social risk factors which can be address through Care Management

Provides organized coordination and management of medical, mental health, substance abuse, and social service needs to Medicaid individuals with chronic conditions. The Care Manager will assist the individual in coordination of care through effective communication with providers, navigation of the complex healthcare system and linkage to resources for finding affordable housing, food, clothing, childcare and transportation.
