Eligibility

Individuals with disabilities who:
  • have Medicaid
  • reside in New York State
  • willing and able to self-direct their services, or have a designated representative who is willing to do it
  • need assistance with daily living activities, home health care, or skilled nursing tasks as part of a long-term health condition or disability

Hours

Administrative hours: Monday - Friday, 8 AM - 4 PM Service hours available 24 Hours / 7 Days
