Provides case management, coordinated home care services including nursing care and management, physical therapy, occupational therapy, respiratory therapy, social work services, speech therapy, nutritional counseling, personal care, and homemaker/housekeeper services.
The program can also coordinate a personal emergency response system, Meals on Wheels, respite care, adult day health care, and social day care. Visits by specialists such as podiatrists, dentists, and ophthalmologists are arranged as needed.
Specialized services include: HIV/AIDS Home Care Program and Traumatic Brain Injury Waiver Services.
