Eligibility
Individuals must:
1) Currently have active Medicaid
2) Have two or more chronic health conditions, OR be living with HIV/AIDS, OR have a serious mental illness OR have sickle cell disease
3) Be appriopriate for Health Home services by having identified risk factors
