Provides the following services:

Information and referral on all services
Long-term care needs of the elderly
In-home assessment of elderly clients and development of specialized care plan
Case management to link clients to needed services
Advice for clients and family

Also provides home health services and adult day care to eligible clients as funding permits; may be able to share cost for home health and adult day care services. Caregiver Resource Center has been developed to offer support, information, and services to those who care for seniors and to seniors who care for family members who are under the age of 18.
