Description
Provides case management and supportive counseling for people diagnosed with or chronic disease. Clients can access transportation to eligible services, integrates linkage, helps stay connected to care, and treatment adherence services into a continuum of HIV primary care Retention adherence Program (RAP) and Medical Case Management work along side each other to make sure clients have access to connection with services needed to stay healthy.
