Children ages 5-17 years old who reside in Suffolk County and have been diagnosed with a serious emotional disturbance and require Case Management assistance. (Eligibility and referral to Supportive Case Management is done through the Single Point of Access program.)

Provides case management services to children, adolescents and their families to maximize each youth's potential for growth and emotional stability. Includes an assessment of the child/adolescents strengths and needs and development of a service plan focused on mental health, health, education, recreation, self-care and various other areas related to the young person's functioning. Services include linkage, coordination of services, advocacy and education. Evening appointments are available.
