Application process
Call or walk in. Medical Van: Appointments must be scheduled at least three weeks in advance (first come first served) until schedule is full. For Senior Discount Card, fill out and return application form to the office by mail or fax as indicated on the form. Description
Implements services to meet the social, economic, and physical needs of senior citizens, improve their quality of life, and enable them to remain active and viable members of the community. Services include: - Medical Transportation - Senior Discount Card - Monthly newsletter - Information and referral services - Recreational and leisure programming, including outings and day trips Also operates two Senior Centers in the City of Buffalo: Autumnwood Manor Senior Citizen Center and Summer/Richmond Senior Citizen Center (see separate listings).
