Buffalo City - Division of Disabled Parking

To be eligible for a permit:

• Must have doctor approval; original copies only.
• Must bring a copy of a state non-drivers Identification card or a copy of a drivers license.
• If someone other than the applicant is bringing in the application, written consent from the applicant is needed.

Monday-Friday, 8am-4pm.

Applications can be mailed or submitted in person. The applicant completes part 1 of the application, and their doctor completes part 2. To receive an application:

• Download an application from the DMV website: https://dmv.ny.gov/forms/mv6641.pdf
• Call to request an application from the office.
• Stop by the office.

To renew a permit, contact the office (by phone or in person) the month that the permit expires. Temporary permit renewal must be approved by the applicant's doctor.
