Eligibility
Individuals who are 18 and older, enrolled in Medicaid and have two chronic conditions, HIV/AIDS, one or more serious mental illnesses, and have significant behavioral, medical or social risk factors which can be address through Care Management.

A 'Health Home' is a network of support services for Medicaid recipients in the community. Health Homes provide Care Management services to ensure all those involved in an individual's care are working together and sharing the information necessary to support a person's recovery. This means coordinating not just medical, mental health, and substance abuse services, but the social service needs of the individual as well. A Health Home can help individuals navigate the complex healthcare system, link to resources for finding affordable housing, food, clothing, childcare, and transportation, and advocate on behalf of the individual to ensure they're receiving the care they deserve.
