Description

The North Country Chronic Disease Coalition’s goal is for each Health System Service Area to have developed and started implementation of a Community-Clinical action plan. These plans will create and/or increase awareness of health-promoting resources that providers can refer patients to for community-based chronic disease risk reduction activities and support.
