Eligibility

Families who receive TANF, SSI, participate in the foster care system and/or are homeless are categorically eligible for the program. Families with an annual income at or below the HHS Poverty Guidelines are also eligible. In addition, at least 10% of enrollment is available to children with disabilities. Call to make an appointment to complete an application. Documentation required: child's birth certificate, proof of current household income, current physical and immunization records, and medical insurance card.
