Provides a one stop facility and assists individuals while in the facility to receive services they require. Service also help in getting medical and behavioral health services. These services, known as the Care Coordination Team, consists of physicians, clinicians, nurses, dentists, outreach workers, care managers, and MSCs, and individual's circle of support.
