Individuals between the ages of 16 years and 30 years that have recently experienced mental health issues for over a week but less than 2 years. Must be willing to work with a diverse team of healthcare professionals.

An innovative outpatient treatment program for adolescents and young adults who may be experiencing mental health issues. OnTrackNY helps these individuals achieve their goals for school, work and relationships. Individuals will have access to a Psychiatrist, Nurse, Therapist, Peer Specialist and Education and Employment Specialist when enrolled in OnTrackNY.

Services are available for all ages and are offered on-site at OMH licensed clinics as well as in home and community settings.
