Eligibility

Individuals must be at least 18 years old and live independently in the community; - individuals must be eligible to receive OPWDD service coordination services; - individuals must be living in the community in a non-certified residence alone, with a spouse (significant other) or roommate(s), as the head of household with dependent children, or with a live-in caregiver; - the individual may not live with family or others as a dependent
