Individuals who have NYS Medicaid eligibility, have a Care Manager and qualify for OPWDD services.

  • Provides a program that allows Medicaid eligible individuals with developmental disabilities to receive services at home and in their community, instead of in nursing homes or institutions. Habilitation services provide staff that assist individuals with developmental disabilities in gaining the skills that they need to live independently.
Skills taught that are needed to live independently include:
  • Maintaining a home
  • Participating in recreational and social functions
  • Socialization skills
  • Exercising choice and self-determination
  • Implementing daily practices to promote behavioral and physical health
  • Communicating effectively
  • Exploring new and practical experiences
  • Engaging in and contributing to the community.
The program also allows the individual to set their own goals.
