Primary care services for veterans in Cortland and Tompkins Counties, including: Mobile Outreach and Transition Team (MOTT): short-term case management and therapy to veterans to remove barriers to treatment and help them identify their strengths, resources and community supports. Helps veterans determine eligibility for services, connects them with those services and ensure they receive appropriate care health care: primary health care services, routine diagnostic procedures and dietary services at the clinic. laboratory: blood drawing and analysis prescriptions: routine prescriptions via mail order; emergency medicines provided behavioral health care servicesHandicapped accessible, free parking at facility.
