Serves technology dependent, medically fragile and chronically ill children by offering support to families caring for their children at home, in a hospital or in a specialized skilled nursing residential facility. The primary services include: case management for the Care at Home Program, respite relief, Angela's House (a home for medically frail children), reimbursement program for services and goods which cannot be funded through other sources, informational and referral
