help in understanding treatment, obtaining and maintaining benefits/entitlements and navigating health care systems for individuals with HIV and other chronic illnesses health literacy, including treatment adherence and chronic illness management education referrals to medical care and mental health services referrals to services, such as housing, emergency financial and legal assistance, benefits/entitlements, educational/training opportunities, medical transportation, etc.
