Application process
Must be able to live independently. Priority given to Nassau County residents. Must have diagnosis from certified NYS psychiatrist and be referred through the Single Point of Access system (SPA). Interested persons should contact the Single Point of Access, SPOA of Nassau County at (516) 227-7057.
