Eligibility

Birth to five years of age. Must have a confirmed developmental delay, have a medical or physical diagnosis with a high probability of developmental delay, have been screened and found to need further evaluation, suspected of or at high risk for developmental delays in one or more of the following domains: Cognitive, communication, motor, social/emotional and adaptive (self-help). Refer your own child to Early Intervention services, or a health care or child care professional can do it with your permission.
