Eligibility
Coordinated Entry uses the HUD definition of homelessness. A homeless person is someone:
- Living in a place not meant for human habitation
- Living in emergency shelter (including domestic violence shelter)
- Living in transitional housing for homeless persons, with an imminent exit date
- Exiting an institution where they temporarily resided for up to 90 days (and were in a shelter or a place not meant for human habitation immediately prior to entering that institution)

Intakes can occur via phone call or in-person at the location below:
-The Community HUB location in this Continuum of Care is the downtown library. A Resource Room is located in the basement of the Buffalo and Erie County Library Central location at 1 Lafayette Square, Buffalo, NY 14203. This location can be contacted at 716-367-8188. The Library Resource Room will be open Monday-Friday, 8:30am-4:30pm. in the City of Buffalo, 14204.

Offers 58 scattered site, furnished apartments to chronically homeless individuals as part of a Housing First Model. The Housing First Model does not put conditions on housing; treatment and sobriety are not conditions, and income and ID are not required.
Low to medium demand supportive case management is provided. The resident will be assisted with obtaining food, appropriate clothing, identification, benefits, medical services, and follow ups. Opportunities for work activities to develop skills and income potential are made available.
