Erie County residents 55 and older who are eligible for nursing home care, but choose to remain at home with assistance; should be expected to need long term care services for at least 120 days.

A comprehensive managed long term care program for older adults who wish to live at home but need assistance in order to maintain health, complete day to day activities, and remain safe at home for as long as possible. Primary and specialty medical care, adult day center, meals, prescriptions, hospitalization, and emergency services provided. In-home support, physical and occupational therapy, and transportation are also available.
