Eligibility
Must be living in a nursing home for more than 60 consecutive days.
Wants to live independently in the community and can do so safely.
Are moving to a qualified community residence such as: owned or rented home, residence in the community with no more than four unrelated individuals residing at, rented apartment with an individual lease.
Are a Medicaid recipient.

- Provides transition specialists to help people move from long-term care facilities back into their homes and communities. A Transition Specialists can work with you and your family members to help you determine what services you will need to live on your own, as safely and independently as possible. We will also work with you to help you figure out how to get these essential services; such as:
- affordable and accessible housing
- the information and skills you'll need once you return home
- any other additional services that you may benefit from (such as service coordination or case management).
