Eligibility
Must be at least 2 years of age.
Meet New York State Medicaid eligibility requirements.
Have an assigned Care Manager (CM).
Complete the Front Door process and qualify for OPWDD services (Office for People With Developmental Disabilities).
Obtain a start-up broker, a Fiscal Intermediary (FI), and a Circle of Support (COS).
Opt for HCBS waiver services instead of institutional care.

- Promotes independence, as the Fiscal Intermediary, for individuals with developmental disabilities by allowing individuals to create and manage their own budgets, hire their own support staff, and engage in activities that will enhance their ability to live and work in the community.
- Provides individuals with developmental disabilities choices to control their care including:
- Employer authority that enables individuals to hire, manage, and dismiss workers;
- Budget authority that provides the individual with a flexible budget to purchase the supports and services needed to live independently;
- Daily activities such as paid or volunteer work, continuing education, and recreation;
- Support: family, coworkers, friends, support staff;
- Where to live
- Set the rhythms of daily life: what time to eat, when to go to sleep, etc.
