Eligibility
Individuals who are at least 60 years old and:
- In need of assistance with an Activity of Daily Living or two Instrumental Activities of Daily Living;
- Able to be maintained safely at home; and
- Not eligible for the same services under Medicaid.

Provides Personal Care Level I and Personal Care Level II services. Level I services consist of assistance with Instrumental Activities of Daily Living (IADLs), such as housekeeping, cooking, and shopping. Level II services consist of assistance with both IADLs and Activities of Daily Living (ADLs), such as dressing, bathing, and moving in and out of a bed, chair, etc. Also provides personal emergency response buttons.
