Provides financial assistance to Columbia and Greene County residents experiencing hardship due to illness or injury. Financial assistance to offset expenses associated with medical care such as co-pays and co-insurance, insurance premiums, and prescription drug costs as well other expenses such as rent, utilities, and vehicle gas; and, information and referral to other resources.

THE HEALTHCARE CONSORTIUM

The mission of the Healthcare Consortium is to increase access to healthcare and support the health and well-being of the residents in Columbia and Greene Counties. We address the barriers--whether they be physical, financial, or practical--that people confront when trying to get and stay healthy. Services provided include: non-emergency medical transportation through our Children and Adults Rural Transportation Service (CARTS); health insurance enrollment services through our Navigator Program; information, referral and assistance for individuals in need of long-term services and supports through our NYConnects program; access to no or low-cost prescription medications; financial assistance for those who are experiencing hardship due to illness or injury; and what we call "way-finding," helping people to find their way through the healthcare maze.
