Provides comprehensive care management to people with complex behavioral and medical diagnoses with the goal of increasing adaptive functioning and decreasing their need for emergency room and inpatient services. Service include outreach, engagement, health risk assessment, care planning, treatment team coordination with behavioral health and medical providers and linkage to clinical services and community supports.
