Families who receive TANF, SSI, participate in the foster care system, and/or are homeless are categorically eligible for the program. Also eligible are families with an annual income at or below the HHS Poverty Guidelines.

Application process
Call to make an appointment to complete an application. Documentation required: child's birth certificate, proof of current household income, current physical and immunization records, and medical insurance card.
