Individuals must:
- receive Medicaid, Medicaid insurance plans, or be eligible to receive Medicaid
- have been diagnosed with two or more chronic physical conditions, HIV/AIDS, or serious, persistent mental illness
- have significant behavioral, medical, or social risk factors
