Youth ages 12 to 20 in the juvenile justice system who are returning to Albany County; Youth from other counties may be served at the request of DJJOY

Provides mental health and case management services to youth ages 12 to 20 years in the juvenile justice system who present with a full range of mental health diagnoses and await release from a juvenile justice facility. Services begin prior to a youth's release from the facility and continue in the home after release.
