Application process
For long term help the application will depend on the drug and the company. Call the Community Health Advocate for help starting the process. For short term prescription help, Client Services Application must be completed and signed by individual making request or by an individual authorized to sign on the person's behalf. Obtain an application by downloading it from www.chenangohealth.org or by calling the agency at 607-337-4128
