Provides a Health-Related Social Needs (HRSN) Screening, a standardized, 1:1 assessment used to identify whether a Medicaid member has unmet social needs that impact their health. Screenings are provided by a Social Care Navigator, a trained professional who guides Medicaid members through the process of addressing their identified social needs. Navigators may provide Eligibility Assessments, Social Care Plans, referrals, care coordination, and follow-up.
