Applications are available at both DHS locations (691 St. Paul Street and 111 Westfall Road), many community service agencies, on this website http://otda.ny.gov/programs/applications/ or can be mailed by calling the appointment line. Emergency assistance for food or rent must apply in person at 691 St. Paul Street.
