A special Medicaid program the allows medically fragile children to be cared for at home by providing case management including evaluating and assisting with a child's therapeutic, medical, educational, and psychosocial needs, working with medical equipment and home care personnel and assessing resources to provide appropriate care within the home setting. Home adaptation and vehicle modification are also available to improve m ovement and/or safety for the child at home and to improve access from home to the community. Also trains families in the necessary equipment and care techniques. Call for additional information.
