Individuals who are homebound and/or ages 60 years and older who are:
- incapacitated due to accident, illness, or frailty
- lacks the support of family, friends, or neighbors
- unable to prepare meals because of lack/inadequacy of facilities, or inability to shop, cook or prepare meals safely, or lack knowledge or skill
