COORDINATED ENTRY SYSTEM FOR HOMELESS SERVICES | COORDINATED CARE SERVICES INC.

Eligibility
Homeless by HUD's category 1) Homeless living in a place not meant for human habitation(i.e. street, car, or an abandoned building) or a homeless shelter. Or Category 4) fleeing a domestic violence situation.
Hours
Main Office: M-F 9:00AM-5:00PM.On line referrals can be made 24 hours.
Application process
Referral needed. Can be completed by a homeless service provider that is working with the client or another provider that has an on going relationship with the person.
Fees
No fee for services.

  • Provides a unified response to move persons quickly out of homelessness and into permanent housing. Designed to improve fairness, create easier access to resources, and increase the efficiency of the local crisis response system to address homelessness.
