Eligibility

Families of children from birth to 3 years, in which the infant/child meets any of the following:
  • born after a pregnancy of less than 33 weeks
  • birth weight at or below 3 pounds, 5 ounces
  • spent 10 days or more in a neonatal special care unit
  • born to mothers less than 16 years old
  • diagnosed with a health problem that might affect development
  • parent or doctor suspects a potential for slow development
