Eligibility
Families of children from birth to 3 years, in which the infant/child meets any of the following:
  • born after a pregnancy of less than 33 weeks
  • weighs less than 3 pounds, 4 ounces
  • spent 10 days or more in neonatal or special care unit
  • born to mothers less than 16 years old
  • has been diagnosed with certain medical problems at, or shortly after birth, such as heart and respiratory problems
  • diagnosed with special needs and/or potential developmental problems
