Works with youth who are on probation and have been identified as high risk. Offers in-home services consisting of a social worker for in-home therapy, a family educator who administers evidence-based family skills programs with parents and youth, and a care coordinator who addresses needs identified by the family and team. Maintains probation and school contract to facilitate youth progress. Arrangement of psychiatric evaluations when deemed necessary. Referral required.
