Description
Provides comprehensive assessments of an individual's home health care needs, coordinates the most appropriate effective services, and provides case management. Designed to help health impaired Erie County residents remain in their homes. Provides linkage for clients with home care needs including personal care, meals, and housekeeping. Screens for Medicaid eligibility for home care.
